About the Role

We are seeking a dedicated Complex Transitional Care Nurse to join our Community Transition team. In this remote role, you will be instrumental in developing personalized, evidence-based care plans for individuals with complex behavioral and physical health needs. You will coordinate care, collaborate with multidisciplinary teams, and ensure members receive comprehensive support for optimal health outcomes.

What You'll Do:

  • Develop personalized care planning strategies through thorough assessment of medical history, social circumstances, and individual needs.
  • Coordinate care for individuals with co-occurring behavioral health, physical health, or multi-morbid conditions.
  • Collaborate with internal staff across various disciplines to facilitate integrated care.
  • Monitor care plans, service delivery, and the health and safety of members.
  • Perform clinical functions of discharge/transition planning and diversion, including assessments and referrals.
  • Provide education on available services, treatment options, and community supports.

Qualifications:

Required:

  • Current Registered Nurse (RN) license by the North Carolina State Board of Nursing.
  • Minimum of one (1) year of experience as a Registered Nurse.
  • Valid driver's license.
  • Must reside within Trillium's South Central Region (Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne counties).
  • Ability to travel within the catchment area as required.

Preferred:

  • Experience working with individuals with Behavioral Health (BH), Mental Health (MH), Substance Use (SU), or Intellectual/Developmental Disabilities (IDD) populations.
  • Knowledge of Quality Management (QM) and Utilization Management (UM) procedures.
  • Experience using data analytics for population health management.
  • Experience assessing and coordinating care in settings such as adult care homes, family care homes, or private residences.
