### About the Role
Chronic Care Staffing is looking for a compassionate and detail-oriented Remote Care Coordinator - CMA/RMA to support patients managing chronic conditions. This role involves proactive outreach, education, and care coordination, acting as a key point of contact for patients and a communication bridge for providers.
### Responsibilities
* Conduct structured monthly outreach calls to monitor patient progress and engagement.
* Verify medication adherence and assist with health risk assessments.
* Support Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) initiatives.
* Document patient interactions, care updates, and clinical information accurately within EHR systems.
* Coordinate communication between patients, families, providers, and internal care teams.
* Identify barriers to care and escalate patient concerns when necessary.
* Educate patients on care plans, treatment goals, and available support resources.
* Maintain HIPAA compliance and CMS documentation standards at all times.
### Requirements
* Residency within the PST or MST time zone.
* Active, unencumbered CMA or RMA certification.
* Current Basic Life Support (BLS) certification.
* Recent experience in a clinical, care coordination, or chronic care management setting.
* Strong proficiency with EHR systems and Google Suite.
* Dedicated, HIPAA-compliant home office with reliable high-speed internet.
* Excellent communication, organization, and patient engagement skills.
