Founding Engineer (Python/AI)

Doctors spend up to 68% of their time on administration instead of patient care. Sikia AI is building AI agents to free up doctors, allowing them to focus on people, not paperwork. Our first product, an AI assistant for documentation and billing, is already in use in clinics and practices across Germany, documenting thousands of patient conversations weekly. The product works, doctors love it, and we're growing fast. Now, we're seeking our first Founding Engineer to take us to the next level.

Responsibilities

• Build and take end-to-end ownership of central systems alongside our CTO; your code will be live in clinics within weeks.
• Engage directly with doctors who use the product to gather feedback and drive improvements.
• Develop and manage the Python backend powering our real-time medical documentation.
• Implement live transcription of doctor-patient conversations, integrating multiple STT providers, quality monitoring, and fallback mechanisms.
• Manage our Applied AI core, including LLM evaluation pipelines, prompt optimization, multi-provider routing, and fallback chains, considering cost and latency for large-scale AI operations.
• Maintain multi-cloud production infrastructure ensuring reliability in clinical use.
• Lead greenfield projects defining the future capabilities of our AI doctor assistant.
• Contribute full-stack when necessary, with a focus on backend and AI, while supporting the Flutter (Windows and Web) product base.

Qualifications

• Proficiency in Python: Minimum of three years of programming experience, with a strong command of Python as our core language.
• Builder Mentality: You build and prototype, bringing personal projects to life and aiming to leave your mark on a product.
• End-to-End Ownership: Experience taking real systems from inception to delivery, building reliable solutions.
• High Initiative: Proactively identify problems, propose solutions, and deliver them.
• Applied AI Passion: Experience with modern AI tools and a deep desire to dive into Applied AI (evaluation, prompting, routing, real-time pipelines), whether you're already experienced or eager to learn.
• Founder Mindset: Think like a co-founder, question approaches, identify opportunities, and help elevate Sikia.
• Empathy: Demonstrate empathy for the doctors, patients, and healthcare professionals we serve.
• German Language: Conversational German proficiency.

• Advantageous: Dart and Flutter (especially Windows Desktop), strong product sense, background in Computer Science, STEM, or Medicine, or a self-taught individual with significant delivered projects.
