Case Coordinator - Family Supports

About the Role

VON is seeking a dedicated Case Coordinator to join our Family Supports program. This full-time, regular position involves providing comprehensive client assessments, developing individualized care plans, and supporting clients and caregivers in managing their care budgets and resources. You will work within a framework that promotes client self-management and adheres to VON policies and standards.

This role is crucial in developing and supporting individual support plans for clients with developmental disabilities who are receiving residential support from the Family Home Program. Services are funded by the Ministry of Children, Community and Social Services.

Job Summary

The Case Coordinator is responsible for assessing the needs of individuals requiring support, developing comprehensive therapeutic rapport, and ensuring a high-quality residential service. You will identify and prioritize environmental risks to client health and safety, facilitate access to identified services, and make referrals to community and health resources as needed. Collaboration with partners in care is essential to ensure a shared understanding of client needs and appropriate interventions.

Key Responsibilities

  • Interpret and complete standardized assessment tools (e.g., interRAI CHA, Supports Intensity Scale, Application for Developmental Services).
  • Conduct client assessments using conversation-based methodology.
  • Develop and maintain a comprehensive therapeutic rapport with clients.
  • Identify and prioritize environmental risks related to client health and safety.
  • Develop comprehensive care plans based on clinical assessments.
  • Facilitate client access to identified services and navigate appropriate resources.
  • Make referrals to community and health resources.
  • Consult with partners in care (with client consent) to ensure shared understanding and coordinated services.
  • Monitor client progress and adjust care plans as required.
  • Review client care plan expenditures and support clients in managing their budgets, advocating for additional resources when necessary.
  • Collect and report relevant data on program outcomes to the Manager.
  • Adhere to privacy policies and standards for Community Support Service or Developmental Services agencies.
  • Represent VON professionally to external stakeholders, community agencies, and service providers.
  • Mediate and problem-solve with multiple parties in various contexts.
  • Coordinate and provide Family Home support for a caseload.
  • Ensure referrals and support are provided in emergency and urgent situations.
  • Participate in the screening and matching of new Family Home Providers.

Common Responsibilities

  • Promote the goals and values of VON as an integrated community care provider.
  • Ensure a safe and healthy workplace in accordance with provincial Occupational Health and Safety Act and Regulations and VON's Safety Management System.
  • Abide by all VON policies and work practices.
  • Maintain confidentiality and protection of personal information.
  • Work collaboratively with other staff in a team approach.

External and Internal Relationships

  • Frequent engagement with Administrative Support, Community Support and Home Care program Coordinators and Manager.
  • Networking and collaboration with regional partners, Health Care providers, and non-traditional providers and agencies.

Education, Designations and Experience

  • University Degree or College Diploma in Health or Social Services field.
  • Minimum of 3 years of related experience in a home and community care organization or social services program, including case management and building external relationships.
  • Demonstrated experience in completing validated common assessment tools and maintaining competency.
  • Broad working knowledge of community resources within the defined catchment area.
  • Knowledge and skills in comprehensive assessment and the ability to maintain proficiency.
  • Experience working with volunteers and seniors is an asset.

Skill Requirements

  • Strong attention to detail and accuracy.
  • Skilled in communication (verbal, written, listening), rapport building, and relationship management.
  • Computer proficiency in Windows OS and Microsoft Office programs.
  • Analytical skills to understand and interpret client assessment data.
  • Ability to effectively manage multiple tasks, prioritize, meet deadlines, and work well under pressure.
  • Knowledge of social and developmental service sectors.
  • Demonstrated skills in leadership and team building.
  • Ability to educate clients and families on available program resources.
  • Strong time management and organizational skills.
  • Ability to work independently and as part of a team.
  • Ability to accept and facilitate change.
  • Ability to maintain confidentiality.

Other Requirements

  • Available for weekend assessment coverage.
  • Must have access to a personal vehicle for business use, a current driver’s license, and proof of vehicle insurance.
  • A current and original satisfactory Criminal Records Check is required.
  • A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • Ability to work flexible hours, including evenings and weekends, to meet client and family needs.
  • Ability to respond to emergency client situations 24/7 (on call).
  • Use of Personal Protective Equipment (PPE) may be required.

Working Conditions

This role requires a detail-oriented approach in a dynamic environment. Physical activity may include lifting, carrying, bending, reaching, kneeling, and other movements requiring good body mechanics. Individuals will walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
