About The Role

Join our innovative Mobile Rehab Therapy Team, focusing on flexibility, patient-centered care, and clinical innovation. This hybrid Physical Therapist role delivers outpatient physical therapy directly in patients' homes, also utilizing telehealth services, with potential for in-clinic support. The ideal candidate is adaptable, passionate about personalized care, and values flexible scheduling, which may include daytime or early evening hours until 6:30 PM.

Responsibilities

  • Demonstrate Northwestern Medicine's values: teamwork, patients first, integrity, and excellence.
  • Provide accurate information and timely updates to patients and customers, addressing concerns promptly.
  • Maintain primary responsibility over the patient plan of care through collaboration with treating PTs/PTAs.
  • Demonstrate teamwork by assisting co-workers, respecting diverse opinions, and acknowledging contributions.
  • Manage work schedule efficiently, completing tasks on time.
  • Contribute to continuous improvement efforts, cost reduction, and service growth.
  • Apply creative problem-solving and diversity in treatment adaptation.
  • Demonstrate essential skill competencies in evaluations, treatment modalities, and equipment use.
  • Perform re-evaluations and adjust treatment plans accordingly.
  • Assume complete responsibility for patient management from initial contact to discharge, ensuring functional progress, safety, and treatment outcomes.
  • Adhere to department policy for evaluations, progress notes, and discharge summaries, documenting in the electronic medical record (EMR).
  • Complete daily notes reflecting patient response, performance, and assessment relevant to the treatment plan and discharge needs.
  • Provide educational materials and methods appropriate to the patient's understanding and motivation.
  • Participate in committees, departmental processes, and special projects.
  • Develop and maintain professional relationships with healthcare team members.
  • Create a culture of respectful communication, listening, and open conversation.
  • Accept constructive feedback and make timely changes.
  • Complete non-patient care responsibilities within established timelines.
  • Contribute to educational efforts of the department/clinic.
  • Participate in student clinical or observation programs as a clinical instructor, mentor, or host.
  • Maintain productivity standards.
  • Demonstrate willingness to flex schedule or assignment to meet patient care needs and attend meetings outside normal work hours.

Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in the State of Illinois.
  • BLS from American Heart Association upon hire.

Preferred:

  • Master’s or Doctoral Degree in Physical Therapy.

Additional Information

  • Coverage Area: Primary coverage includes Streeterville, Lincoln Park, and Lakeview neighborhoods, subject to change based on needs.
  • Schedule: Must be able to work as late as 6:30 PM most days (Monday - Friday). Flexible schedule with a mix of daytime and early evening hours.
