General Practitioner - Chronic Disease Management (Ipswich & Remote)

About the Role:
Are you a GP seeking meaningful, flexible clinical work? Veteran Pathways & Proactive Care is looking for experienced General Practitioners to deliver chronic disease management and structured multidisciplinary case conferencing for veterans and patients with complex health needs. This role offers a unique blend of in-clinic and remote work across two structured days each week.

Key Responsibilities:

• Provide clinical leadership within integrated Chronic Care Teams.
• Conduct comprehensive GP consultations (in-clinic and telehealth) for acute, chronic, and complex conditions.
• Deliver preventative health assessments (cardiovascular, metabolic, mental health, lifestyle risks).
• Undertake chronic disease management (CDM) reviews and care planning (GPMPs, TCAs).
• Lead and participate in structured telehealth case conferences with multidisciplinary teams.
• Manage pathology, imaging, prescribing, and referrals.
• Ensure clinical safety and adherence to escalation pathways.

Work Arrangement:

• Two structured days per week:
• In-clinic day (Tuesday): 8-hour clinic day at our Ipswich partner practice.
• Remote case-conferencing day (Thursday): 8-hour telehealth session from your home office.
• Some flexibility on days to suit your availability.

Veteran Health & DVA Focus:

• Dedicated focus on veteran health and chronic disease management for Medicare and DVA-funded patients.
• Experience with complex, multi-system conditions common in veterans.
• Experience with DVA billing, CVC Program, or veteran health assessments is highly regarded but not essential.

Remuneration:

• First 3 Months (Guaranteed): $240.00 per hour
• After 3 Months: 70% of billings
• Sign-on bonus and anniversary incentive available.

Requirements:

• Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA).
• Fellowship with RACGP or ACRRM (VR GP), OR eligibility for a 19AB exemption in a DPA location (IMGs welcome).
• Experience in chronic disease management, preventative health, and complex care coordination.
• Competence in prescribing, referrals, and investigation management.
• Strong communication and collaboration skills within multidisciplinary teams.
• Suitable home-office environment and reliable internet connection for telehealth.

Desirable:

• Experience with telehealth consultations and digital-health platforms.
• Experience in team-based or multidisciplinary care models.
• Interest or experience in veteran health, DVA processes, or the CVC Program.
• Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs), and chronic disease care planning.

• Ipswich partner practice and remote (home office).
• Ipswich is a DPA-eligible location.
