The Role:
We are seeking a Medical Assistant with a clinical eye and a passion for cardiac care to join our care delivery team. This hands-on remote role involves monitoring patient vitals daily, logging Cardiac Care Management (CCM) time, managing Remote Patient Monitoring (RPM) practice, supporting our virtual cardiac exercise program, and maintaining clinical operations in eClinicalWorks (eCW). This role offers significant clinical responsibility and opportunities for growth.

What You'll Own:

• RPM & CCM Monitoring: Review RPM vitals dashboard daily, identify and act on abnormal readings, initiate patient outreach, escalate urgent concerns, log CCM time in eCW, and ensure documentation is audit-ready.
• Virtual Cardiac Exercise Program: Moderate weekly virtual classes, document attendance, and create group visit notes in eCW.
• Patient Onboarding & Chart Prep: Oversee AI-assisted onboarding, verify intake accuracy, prepare charts in eCW (medications, allergies, history, vitals), monitor device inventory, and confirm RPM device linkage.

What You Bring:

• Medical Assistant certification (CMA, RMA, or equivalent).
• 2+ years of MA experience in a clinical, telehealth, or care management setting.
• Proficiency in eClinicalWorks (eCW) is required.
• Working knowledge of CCM, RPM, or chronic disease management programs.
• Experience in cardiology, cardiac rehab, or cardiac-focused telehealth is a strong plus.
• Familiarity with CCM/RPM CPT coding is preferred.
• Bilingual skills are a plus.
