The Opportunity
Pine Park Health is seeking a Full-Time Physician Assistant to deliver care in our Phoenix, Arizona market. This role is perfect for individuals excited about managing a smaller panel of complex care patients, contributing to a value-based care model, and working with a motivated multidisciplinary team to provide high-quality patient care. Our person-centered care model utilizes care navigators to maximize provider efficiency and focus.
What You’ll Do
- Provide onsite primary care services to older adults residing in Senior Living Communities.
- Develop and execute detailed, person-centered care plans for all primary care patients.
- Collaborate with the Medical Director to identify and implement policies addressing daily practice, culture, or clinical guideline issues, including those related to emerging diseases.
- Conduct monthly Chronic Care Management (CCM) visits, including reviewing and signing off on CCM documentation.
- Lead regular team huddles and provide clinical team leadership.
- Identify opportunities to collaborate with senior living management and care teams to enhance the execution of Pine Park care plans and improve patient outcomes.
What We’re Looking For
- Physician Assistant qualification, with a preference for experience in geriatrics.
- Comfort working in a fast-paced and evolving environment.
- Willingness to provide onsite clinical services across the Central Phoenix/Tempe region.
- A desire to work with complex care patients and support the development of a holistic care model.
- A strong orientation towards collaboration and teamwork, valuing input from all clinical team members.
- Excellent written and verbal communication skills.
- Proficiency in using tech-enabled care delivery tools (e.g., project management, checklist management, remote monitoring) to enhance patient care and team function.
- Flexibility in work schedule with travel required within the Phoenix area, including driving between partner communities as needed.
