The Opportunity
Pine Park Health is seeking a full-time Physician Assistant for our Phoenix, Arizona market. This role is ideal for individuals excited about managing a smaller panel of complex care patients within a value-based care framework. You will collaborate with a motivated, multidisciplinary team, leveraging care navigators to optimize provider time and energy, all while delivering high-quality, person-centered care.
What You’ll Do
- Provide onsite primary care services to older adults living in Senior Living Communities.
- Develop and execute detailed, person-centered care plans for all primary care patients.
- Collaborate with the Medical Director to identify and develop policies addressing practice, culture, or clinical guideline issues, including for emerging diseases.
- Conduct monthly Chronic Care Management (CCM) visits and sign off on CCM documentation.
- Lead regular clinical team huddles and provide team leadership.
- Identify opportunities to collaborate with senior living management and care teams to enhance care plan execution and patient outcomes.

- Willingness to provide onsite clinical services across the Central Phoenix region.
