The Opportunity

Pine Park Health is seeking a full-time Physician Assistant for our Phoenix, Arizona market. This role is ideal for individuals excited about managing a smaller panel of complex care patients within a value-based care framework. You will collaborate with a motivated, multidisciplinary team, leveraging care navigators to optimize provider time and energy, all while delivering high-quality, person-centered care.

What You’ll Do

  • Provide onsite primary care services to older adults living in Senior Living Communities.
  • Develop and execute detailed, person-centered care plans for all primary care patients.
  • Collaborate with the Medical Director to identify and develop policies addressing practice, culture, or clinical guideline issues, including for emerging diseases.
  • Conduct monthly Chronic Care Management (CCM) visits and sign off on CCM documentation.
  • Lead regular clinical team huddles and provide team leadership.
  • Identify opportunities to collaborate with senior living management and care teams to enhance care plan execution and patient outcomes.

  • Physician Assistant, preferably with geriatrics experience.
  • Comfortable working in a fast-paced and dynamic environment.
  • Willingness to provide onsite clinical services across the Central Phoenix region.
  • A desire to work with complex care patients and contribute to building a holistic care model.
  • Strong collaborative skills and a commitment to working closely with a multidisciplinary team, valuing input from all members.
  • Excellent written and verbal communication abilities.
  • Proficiency with tech-enabled care delivery tools (e.g., project management, checklist management, remote monitoring).
  • Flexibility in work schedule with travel within the Phoenix area to partner communities.
