About the Role
As a Community Health Navigator on the Transitions of Care Team, you will provide essential telephonic and in-person patient support to ensure continuity of care after hospital discharge, focusing on social determinants of health.
What You'll Do
- Collaborate within an interdisciplinary team to meet patient needs.
- Conduct outreach to emergency room discharged patients to ensure continuity of care and address social determinant needs.
- Deliver relevant resources to families within your geographic area.
- Coordinate with facility discharge planners to remove barriers to post-hospital care.
- Attend daily team huddles to communicate patient needs and progress.
- Liaise with hospitals and health systems to promote the Imagine Pediatrics program.
- Support patients with in-person/community needs, including pharmacy coordination, PCP linkage, SDOH needs, and home visits.
- Perform other duties as assigned.
What You Bring & How You Qualify
- Bachelor’s degree required.
- Bilingual (Spanish) proficiency is strongly preferred.
- National CHW certification is preferred.
- CPR certification is preferred.
- 3+ years of experience in a healthcare setting (patient-facing hospital or facility role), with ER setting experience being preferred.
- Passion and commitment to reimagining pediatric healthcare and supporting children with special health care needs.
