About the Role
As a Family Resource Specialist, you will provide direct support to patients and families in addressing social needs. You will work collaboratively with community partners and care team members to bridge gaps related to non-medical drivers of health. This role involves building and strengthening community relationships, maintaining a robust network of resources, and offering non-clinical support, guidance, and education to families navigating the challenges of caring for children with complex medical and behavioral health needs.
What You'll Do
- Identify and connect families/caregivers with available community support resources.
- Provide information, referrals, and non-clinical assistance to link families with medical, psychosocial, and basic needs services.
- Empower families through education and guidance to advocate for themselves and navigate systems.
- Assist families in accessing programs and supports for psychosocial needs within Imagine Pediatrics and the broader community.
- Advocate with community agencies on behalf of clients and families to reduce service gaps and remove barriers.
- Collaborate with internal staff and providers for system navigation (eligibility, pediatrics, behavioral health, etc.).
- Assist in developing an internal community resource library (excluding clinical materials).
- Provide linkage to community-based and school-based services.
- Network with community resources to expand knowledge of available services.
- Administratively document Memoranda of Understanding (MOUs) with outside community agencies.
- Develop effective working relationships with interdisciplinary care teams.
- Participate in consultations with an interdisciplinary team regarding non-clinical resources provided to families.
- Perform other duties as assigned.
What You Bring and How You Qualify
- Bachelor’s degree or higher in a healthcare, behavioral health, or social services related field is preferred.
- Community Health Worker certification is highly desired.
- Familiarity with navigating social service agencies.
- Reliable transportation, a valid Driver's License, and proof of current auto insurance are required.
- Experience with resourcing and telephonic services.
- Bilingual Spanish proficiency is preferred.
- Passion and commitment to reimagining pediatric healthcare and supporting children with special health care needs.
