The Opportunity

We are seeking a compassionate and dedicated Per Diem Registered Nurse to join our team in Oregon. This role is primarily remote, with required fieldwork and occasional visits to our Portland or Salem offices. Candidates MUST reside in Oregon. You will play a vital part in administering and ensuring the efficient delivery of Personal Care Services (PCS) and Structured Family Caregiving (SFC), overseeing Personal Care Attendants (PCAs), and ensuring compliance with all regulatory requirements.

Your Day-to-Day:

  • Ensure compliance with all contractual guidelines and federal/state regulations for Medicaid and the Personal Care Services program.
  • Optimize day-to-day PCA operations by analyzing, monitoring, and improving workflows and processes.
  • Provide oversight to PCS/SFC caregivers in collaboration with the Care Manager.
  • Conduct initial home visits to define PCA tasks and develop activity plans.
  • Perform regular evaluations (every 60 days) to assess PCA performance, consumer satisfaction, and adherence to activity plans.
  • Uphold client and employee rights, privacy, and security of protected information in an ethical manner.
  • Foster a positive work environment aligned with FreedomCare's core values.

Education and Experience:

  • Valid and active RN License from the Oregon Board of Nursing.
  • 2 years of recent work experience in direct care of elders or adults with disabilities.
  • Previous experience in a management role.
